HomeCare by JFS is a CHAP accredited program. CHAP (Community Health Accreditation Program) is the leader in improving the quality of community based health care services in the USA. CHAP's goal is for home care to not only prosper, but gain strength in the overall health care industry.

Services

Skilled Nursing Care
Nurses perform medical assessments and treatment in the comfort of one's home. Wound treatment, IVs, and Diabetes instruction are just a few examples of this type of care. Patients must have a referral from their physician.

Physical & Occupational Therapies
Licensed physical and occupational therapists help individuals adjust to changes in their physical conditions due to illness, surgery, or accident. With professional, in-home services recommended by a physician, individuals learn to live again.

Personal Care
Aides assist with bathing, dressing, toileting, walking, pre-poured medications, meal preparation and light housekeeping.

Homemaker/Companion Care
While offering companionship and support, aides help with light housekeeping, grooming, and meal preparation.

Counseling
Licensed clinical social workers help older adults and their loved ones cope with the many facets of the aging process. Counseling may take place either in an individual's home or at JFS' office on Patterson Avenue.

Care Management
Elder care managers coordinate an individual's care when busy and long-distance family caregivers are unavailable. Follow this link for more information on care management.

Telephone Reassurance
A volunteer calls individuals every weekday to assess their well-being and provide conversation. This is a free service to older adults at their family's or their own request.

Friendly Visitor
Volunteers visit homebound and isolated individuals, developing enriching relationships through weekly visits. This service is also free, but limited to Jewish seniors.
