As a caregiver for an ill, elderly, or disabled adult, you make difficult decisions. You don’t have to make them alone. Care Management by JFS provides the guidance and support you need to make the right choices. Whether you live near or far, Care Managers have the resources to help you make tough life decisions and life transitions.
Comprehensive Care Management services include:
- Check-up phone calls and visits
- Transporting to weekly errands and appointments
- Navigating the health care system
- Addressing personal and home safety needs
- Making the transition to a care facility if needed
- Regular communication with family members near and far
JFS Care Managers are trained to assess the needs of elderly and disabled adults and create individualized care plans to help them to live as comfortably and independently as possible.
Bill Paying by JFS helps adults pay their bills on time and keep their finances in order. Services might be routine like helping individuals manage their bills, write checks, and balance their accounts; or as comprehensive as JFS serving as a legal authority such as Power of Attorney, Guardian, or Representative Payee. Clients can choose to have bills sent directly to JFS and managed according to client preferences and needs, or a JFS bill payer can assist with bill paying and financial tasks in the client’s home.
Meaningful Moments by JFS is a Montessori based program utilizing reality based every day activities to meet social and emotional needs of loved ones living with memory loss.
IFS by JFS is a NORC program. NORC stands for Naturally Occurring Retirement Community-meaning, people have "aged in place" over the years in their own neighborhoods. Studies show that aging "in place" is the preferred choice. The trouble is, our bodies change with age and don’t perform as before. We may need a little extra "something" to stay home. This something may be a ride to the store or help lifting groceries out of the car. IFS is designed to provide this "extra something."
Caregiver Support Group by JFS is a psychoeducational group that offers a comfortable, confidential environment for caregivers struggling in their role. Its goal is to help family members, friends, partners, neighbors and caregivers get the practical information and emotional support they need to lighten the demands of caregiving. Participants learn from and support each other as well as develop tools for coping. Monthly topics may include care of self, loss and grief, community resources and boundaries and communication. This service is free of charge to the community. The group meets the third Thursday of every month from 2-3:30 p.m. at JFS.

While Care Management by JFS is largely a fee for service program, fee subsidies are available to current clients who have spent down their savings and for clients in need of care who cannot afford full fee. A JFS care manager will be happy to discuss this with you.
